Job Responsibilities Include: Oversees and directs, the overall planning of the psychiatric/medical care and treatment of clients, and has the final decision making authority on clinical matters effecting their care and welfare. Advocates on behalf of client, and links with systems and services to meet identified needs. Monitors revenue and expense reports of program and institutes corrective action as appropriate to ensure compliance with budgetary goals. Monitors productivity of program staff on an ongoing basis and institutes corrective action to ensure compliance. Ensures programmatic goals and objectives are met and institutes corrective action. Executes aspects of administrative responsibilities (timesheets, time off requests, project timelines, etc.) in a detailed and timely manner. Provides direct diagnostic assessments and evaluation and implements treatment plans for clients as required. Completes all documentation (clinical documents, service worksheets, time sheets, reports, grants, etc.). Assures appropriate documentation for contract and system requirements. Trains and educates staff on timely and accurate data submission, process completion and policy protocols. Provides clinical in-service training to center professionals and paraprofessional staff as requested. Reviews clinical records for quality, quantity and performance; advocates on behalf of client and links with systems and services to meet identified needs and provides feedback to staff. Participates in and co-chairs Client Care Committee

Must be able to complete a level II background screening and pre-employment drug test in compliance with Florida Law.

In partnership with law enforcement, local school districts, Alachua County Crisis Services, and other key stakeholders, the program provides collaborative, no wrong door approach to mobile crisis services for young people. Meridian and our partners will implement five Mobile Response Teams (MRTs) within a 11 county in North Florida. Each of the five teams will provide 24/7, on demand crisis intervention and related services for youth and young adults 25 years of age and younger and their families/caregivers.  Services will be provided in the setting in which the behavioral crisis is occurring (e.g. home, school, community.

Position Summary:
This is an independent professional position performing assessments, service brokerage, and advocacy services for persons up to the age of 25 that experiences a behavioral health crisis that meets criteria for care coordination as a response through the Mobile Response Team. Care Coordinators works closely as a part of the Mobile Response Team staff assisting when there is a crisis call, responding when needed as an on-call advocate. The Care Coordinator identify and engage eligible individuals in services, with the goal of reducing admissions to the CSU and assuring follow-up appointment within 72-hours. The Care Coordinator links clients with community, natural, and professional support systems to meet socioeconomic and physical and behavioral health care needs, and provides psychoeducational interventions to assist in recovery and wellness. Care coordination is intended to be short term and intensive. The work is performed with a high degree of independence; however, work is reviewed through conferences, supervision and required reports.  Work is performed in the community, schools, hospitals, client homes, CSU and Detox, and Meridian offices and clinics.

Care Coordinator – New Mobile Response Teams

Mobile Response Team (MRT)

In partnership with law enforcement, local school districts, Alachua County Crisis Services, and other key stakeholders, the program provides collaborative, no wrong door approach to mobile crisis services for young people. Meridian and our partners will implement five Mobile Response Teams (MRTs) within a 11 county in North Florida. Each of the five teams will provide 24/7, on demand crisis intervention and related services for youth and young adults 25 years of age and younger and their families/caregivers.  Services will be provided in the setting in which the behavioral crisis is occurring (e.g. home, school, community.

Position Summary:
This is an independent professional position performing assessments, service brokerage, and advocacy services for persons up to the age of 25 that experiences a behavioral health crisis that meets criteria for care coordination as a response through the Mobile Response Team. Care Coordinators works closely as a part of the Mobile Response Team staff assisting when there is a crisis call, responding when needed as an on-call advocate. The Care Coordinator identify and engage eligible individuals in services, with the goal of reducing admissions to the CSU and assuring follow-up appointment within 72-hours. The Care Coordinator links clients with community, natural, and professional support systems to meet socioeconomic and physical and behavioral health care needs, and provides psychoeducational interventions to assist in recovery and wellness. Care coordination is intended to be short term and intensive. The work is performed with a high degree of independence; however, work is reviewed through conferences, supervision and required reports.  Work is performed in the community, schools, hospitals, client homes, CSU and Detox, and Meridian offices and clinics.
